Options
Basic Dental
• Pays a benefit toward essential care such as an annual exam and cleaning, fillings, X-rays, simple extractions, and fluoride and sealants for children under the age of 14.
Basic & Enhanced Dental
• Includes the Basic Dental option or you can choose to have enhanced coverage. Enhanced Dental pays a higher benefit on fillings and extractions, even paying on procedures such as crowns, bridges, and orthodontics.
Voluntary Dental
• Pays a benefit toward essential care such as an annual exam and cleaning, fillings, X-rays, simple extractions, and fluoride and sealants for children under the age of 14. It also provides select enhanced services.

Contact your administrator to see which of these plans are available to you. If your ministry does not have 100% participation in this dental group insurance, your ministry can still offer you the Voluntary Dental option.
Types of Coverage
Coverage levels under the plan options depend on the type of care you receive. The types of care are defined as follows:
Type A: Diagnostic and Preventative
Covered at 100% with waived deductible and includes regular dental check-ups and cleanings
Type B: Basic Services
Includes fillings, anesthesia, and nonsurgical endodontic and periodontic care
Type C: Major Services
Not covered under the basic option, Type C includes surgical endodontic and periodontic care under the Voluntary option and pre-diagnostic tests (oral cancer screening)
Orthodontia
Covered for children and adults under the Enhanced option, and for children under the age of 22 under the Voluntary option

FAQ
Can I participate individually in insurance if my church does not?
No. Your ministry organization must agree to participate in one or more of the products offered by Geneva for you to participate in them.
How do we add insurance benefits?
You can add insurance benefits for your church/ministry staff by following these simple steps:
Step 1: Visit our My Benefits page to download enrollment instructions and complete two forms:
A) Group Insurance Adoption Agreement (IAA). This allows you to select which insurance products your organization will offer full-time employees.
B) Benefit Plan Enrollment Form. This provides us with the information needed to set up an account in SmartBen (our on-line benefits administrator).
Step 2: Request a secure email link by emailing us at enrollment@genevabenefits.org.
Step 3: Submit forms through secure link.
When are benefits effective?
Benefits are effective the first day of the month following the date of hire. Please do not send contributions before this date.
I’ve worked at my church for years. What happens if I want to add a product or service now?
This needs to be coordinated with Geneva and your employer. There may be voluntary products for which you are eligible. Call us to discuss at 678.825.1260.
Must I be employed by a PCA organization to purchase insurance?
You must be employed full-time, working at least 30 hours a week or more for a PCA church, school or ministry, or like-minded non-profit ministry organization to participate. If you are uncertain if your employer qualifies as a like-minded non-profit ministry, email us at benefits@genevabenefits.org.
